Eight-year-old Adrian Sue Thompson’s last meal at school has become the center of intense scrutiny after her family released cafeteria surveillance video showing her struggle for help. The second-grader at Velma Penny Elementary School in Lindale began choking on April 15 while eating with classmates. Footage captures her coughing, gesturing near her throat, and moving through the room in clear distress before sprinting to the nurse’s office.

Witness accounts and the visual record indicate that initial staff contact treated the episode more like ordinary illness than an airway obstruction. She was guided toward a bin and asked about the restroom before returning and running for medical help. Once in the nurse’s office she received standard interventions, but ᑕriᩚtiᩚcal time had already pᴀssed. She died the next morning after transfer to a Fort Worth pediatric intensive care unit.

Autopsy confirmed accidental choking, and the Texas Rangers found no criminal offense. Parents, safety advocates, and local residents continue to ask whether closer cafeteria monitoring or immediate recognition of the universal choking signal would have shortened the response window. The district has responded with expanded training and additional rescue devices across campuses.
